UW Department Limited and Service Permit Application

UW–Madison departments must use this form to apply for UW Department Limited permits and/or Service permits.

Outside organizations may use this form to apply for UW Vendor permits.

You may apply for multiple permits at the same time.

You may be contacted for more information after Transportation Services receives the application.

Please review the rules, regulations, and policies regarding Department Limited and Service/Vendor permits before applying. Failure to follow UW–Madison Transportation Services policies may result in revoked parking privileges. Responsible parties must inform all users of parking policies and updates.
