Vendor Permit Application

Commercial businesses, private organizations, or contractors providing service or conducting official business at UW–Madison may apply for an on-campus parking permit using this form.

You may apply for multiple permits at the same time.

You may be contacted for more information after Transportation Services receives the application.

Please review the rules, regulations, and policies regarding Service/Vendor permits before applying. Failure to follow UW–Madison Transportation Services policies may result in revoked parking privileges. Responsible parties must inform all users of parking policies and updates.
