UW-Madison Transportation Services has partnered with Lyft, a ridesharing company, to supply Emergency Rides Home. Complete the form below to register for the Lyft version of the Emergency Ride Home program. A limited use ride code will appear on this page and be sent to the provided email once you have submitted the registration form–be sure to save this code.

By signing up through this form you are agreeing to the restrictions of the Emergency Ride Home program as outlined on this webpage and on the Emergency Ride Home program webpage.

By submitting this form I confirm / accept the following terms:*
* I am a UW-Madison or UW Health employee.
* I will use the Emergency Ride Home (ERH) Lyft code for rides from work on days I did not drive to work alone.
* I will only use the ERH Lyft code for rides due to my own illness, the illness of a family member, or due to a personal crisis that required me to leave work.
* I will not use my ERH Lyft code for driver tips.
* I understand I may only take three (3) or fewer trips home in any six (6) month period.
* I understand my ERH Lyft code will expire after six months and I must resubmit this form to get a new code.
